Required Forms

Please submit the following forms to:

UW-Green Bay Special Student Application

Youth Options Program Plan and Report (DPI Form PI-8700A) (PDF)
If you are approved to take more than one course, or wish to provide alternate courses if your first choice is not approved/available, please list all courses on the DPI form in descending order from highest priory to lowest priority.
Official high school transcript (sent directly from high school)

Enrollment Information and Approval Form (PDF)

Once you have applied to the Youth Options Program and your application is complete with the above items, you will be notified of the decision regarding your application.

If you are accepted into the program, you will need to make sure you meet the prerequisites for approved courses, gain appropriate waivers if necessary, and attend orientation in order to register for your desired/approved courses.
