Appendix A - Testing Procedures
Two types of tests are done to meet requirements of 49 CFR Part 382:
Breath Alcohol Test (BAT) - breath is analyzed for presence of alcohol in concentrations of 0.02 or greater.
DOT Drug Screen - Urine specimen is analyzed using the Drug Screen DOT 5 Panel Test. Urine is tested for cocaine, marijuana, opiates, amphetamines, and phencyclidine.
Types of testing:
Pre-employment testing is done for new employees required to have a Commercial Driver License or employees assuming new duties requiring a Commercial Driver License. A DOT Drug Screen is done for pre-employment testing.
Random testing is done to meet compliance with 40 CFR Part 382. Because the number of UW-Green Bay employees required to be tested is small, UW-Green Bay will participate in the Prevea Clinic Consortium for small businesses to meet random testing requirements.
BAT random testing - The Consortium coordinator will draw 7% of available pool on a quarterly basis to meet the minimum requirement of testing 25% of pool annually. Because of the random nature of the process it is possible for an employee to be required to undergo random BAT testing more than once per year.
Drug Screen random testing - The Consortium coordinator will draw 13% of available pool on a quarterly basis to meet the minimum requirement of testing 50% of pool annually. Because of the random nature of the process it is possible for an employee to be required to undergo random Drug Screening more than once per year.
Post-accident testing is required after an accident meeting requirements described under VIII.C. of the UW-Green Bay Alcohol and Controlled Substance Testing Policy. Both BAT and a Drug Screen are required.
Reasonable suspicion/cause testing is done when an employer has a justified reason to believe an employee performing a safety-sensitive function is under the influence of alcohol or drugs. BAT and/or Drug Screening is done at the discretion of the employer.
Return-to-duty testing is used for employees who have been off from work for a period of time. A Drug Screen panel will be done under the authorization of the employer.
Follow-up testing is used for employees who were found to be positive and must return periodically for re-testing according to DOT testing. BAT and Drug Screen panels are done at the authorization of the employer and recommendation of the Substance Abuse Professional.
NOTE: As part of this program we are including the required DOT Physical exam (required every other year). Dr. Jane Sliwinski, Prevea Clinic Department of Occupational Medicine, will do our required DOT Physical exams. Please call 496-4744 to schedule physicals when required.
Testing sites:
Pre-employment testing will be done at the Prevea Clinic Department of Occupational Medicine Dousman site at 1551 Dousman St., Green Bay, WI 54303. An appointment for testing can be made by calling 496-4709. This number will put you in contact with the Dousman site laboratory. Technicians answering phones can schedule you for the appropriate testing when you identify that you are a UW-Green Bay employee wanting to schedule a DOT pre-employment test.
Random BAT and Drug Screening tests will also be done at the Prevea Clinic Department of Occupational Medicine Dousman site at 1551 Dousman St. Employees will report to the Dousman lab promptly when notified of testing requirement. Supervisors will call lab ahead of time to notify technicians an employee will be arriving for either the BAT or drug screening.
Return-to-duty and follow-up testing can be scheduled the same way a pre-employment test is scheduled.
Reasonable cause and post-accident testing that can be done during normal working hours should be done at the Dousman Clinic site. Call 496-4709 to notify of arrival. If reasonable cause or post-accident testing must be completed outside of working hours, the employer has two options:

Green Bay, WI 54303When using St. Mary's Hospital please go to Admitting (not through the Emergency Room). Identify tests required (show the cover letter we have provided) and you will be directed to the laboratory for testing.
Test costs (based on most current prices available):
Breath Alcohol Test (BAT)

Done at St. Vincent's or St. Mary's: $28.00
5 Panel Drug Screen
Done at Dousman Clinic site:
• Negative test: $38.00
• Positive test: $69.00
Done at St. Vincent's or St. Mary's:
• Negative test: $42.00
• Positive test: $73.00
DOT Physical exam
Done at Dousman Clinic site: $53.00
Costs for maintaining an employee in this program:
It is difficult to estimate the cost for maintaining an employee in this program. At minimum an employee is required to have a DOT Physical exam every two years ($53.00 per exam) and a pre-employment drug screening ($38.00 or $69.00 depending on results of exams. All additional costs are variable depending on whether an employee is involved in an accident requiring testing, requires follow-up testing, or is randomly chosen for testing. The random pool process may end up requiring a given employee to submit to more than one BAT and/or Drug Panel screening in a given year or not be tested at all for several years.
All costs for maintaining an employee in this program is responsibility of employing office.
