Below is a form that will notify the Interdisciplinary Studies Program of a change in your Contact Information. Only your first name and last name are required. Fill in only the parts of the form that effect you, e.g., if you are moving and keeping the same phone number and E-mail address, it will only be necessary to complete the change of address portion.

In the Old Information fields type in your old information. (The information that is no longer current and would like changed.)
In the New Information fields type in your new information. (Your updated and new information.)

(Note: This form will not reset your contact information for any other academic and administrative departments of UW-Green Bay, it will only reset your information with the Interdisciplinary Studies Program. To change your contact information for all of the other departments of UW-Green Bay (e.g., Bursar and Registrar) you must complete the form found in the Student Information System under the Personal Portfolio link.)
