Tax Information

UW Green Bay 1098T Forms

The University of Wisconsin System has contracted with a 3rd party vendor, UAS, to provide the 1098T tax document through a website called “Get My Document”.  The 1098T contains important tax information regarding qualified tuition and related educational expenses which may be needed to complete your tax return.  UW-Green Bay reports amounts billed in Box 2 of the form.  The amount shown in Box 2 may represent an amount other than the amount actually paid during the tax year.  Your 2012 1098T will contain charges (less waivers) and scholarships/grants applied to your student account between January 1, 2012 and December 31, 2012Keep in mind that spring 2012 charges were applied to your student account in December 2011 and were included on last year’s 1098T.  Only activity applied during 2012 will appear on your 2012 1098T.

UAS will send several reminder emails during the month of January.  These emails will come to your campus email address from 1098@eforms.uwsa.edu and will contain a link to the “Get My Document” website.  Click on the link in the email to view and print your 2012 1098T document.  Clicking on the link in the email will bring you to the website and auto-fill the security certificate number.  You then enter your name, last four digits of your Social Security Number, and an on-screen security code, to access your document. Be sure to enter your name as it appears in the email. You will also be able to access past year 1098T forms from this site.

If you lose the original email, contact the UWGB Student Billing Office by calling (920) 465-2224 or by email at bursar@uwgb.edu to have the email resent to your campus email account.  These resent emails will come from 1098@eforms.uwsa.edu.

Once you log into their SECURE site you will no longer receive the emails.  From this website you can view and print your 1098T.  Once you view your 1098T on-line, one will not be mailed to you so you will want to print the document from the website.  You can sign onto the “Get My Documents” website as often as you wish using the link from the original email.  If you do not access your 1098T document on-line, it will be mailed to your home address at the end of January.
