Scholarships

For Wisconsin Residents Only.

These DPI scholarships are financial need-based. If the student qualifies for free or reduced lunch AND has a 2.5 grade point average, he/she may be eligible to receive a scholarship. All scholarships must meet this criteria. Scholarships are first come, first serve until funds are depleted.

To apply: Download and print BOTH Part 1 and Part 2 and fill out both forms completely. Also, include a copy of your child's last Report Card and a completed Summer Camp Registration form. Applications will NOT be accepted without ALL FOUR of these forms.

Once completed, please send the DPI Scholarship Part 1 and Part 2, along with a Summer Camp Registration, and a copy of your child's last Report Card to:

DO NOT send the Scholarship Form to the Madison address located on the top corner of the form. It will take your application several weeks longer to reach our office. Scholarships are given on a first come, first serve basis so this would not be in your favor.

**Application MUST be signed by a school counselor or school designee in the administration or district office to prove eligibility for free or reduced lunches. A teacher's signiture is not valid.
