Confidentiality Statement
Maintaining confidentiality of service is an office priority. All counseling and medical records are confidential and
are NOT part of your academic records. Records are not available to anyone without your written consent.
There are rare exceptions to confidentiality. Exceptions include possible suicide, homicide or recent/intended
serious violation of the law. Illegal possession of drugs for personal use disclosed during an appointment will
be kept confidential in accordance with federal law.
