Physical Exam
  For School/Camp/Employment
  1. Make an appointment to see a nurse. Call (920) 465-2380.
  2. Bring form from employer to appointment.
  3. Nurse will complete a basic assessment and schedule you to see the physician.
  4. You will be given the original form and a copy will be maintained in your file.
  5. Cost = $27 (as of 07/01/08)
