The following handbook is a guide to assist UWGB University Supervisors in the supervisory role.
Evaluation Materials
University Supervisors are asked to complete two formal evaluations on his or her student teacher for every nine-week placement followed by a conference with the student teacher. Upon completion of the placement, University Supervisors are also asked to complete a licensure recommendation form. There are various options for submitting these evaluations and recommendations to the student teaching coordinator which are highlighted below.
Electronic Submission
The following links will take you to an electronic form version of the evaluation and recommendation form. Once the form is submitted via the green submission button, the student teaching coordinator will automatically receive the evaluation, and the cooperating teacher will be emailed a copy. Please forward a copy to your student teacher for as well.
University Supervisors are also asked to write a letter of recommendation for their student teachers if appropriate. The following letterhead may be used for a letter of recommendation
