Application Forms
- To apply to a Master's Program you may use our Online Master's Application Form. You should use this application only if you are pursuing a master's degree. See further information below. You should use this form only if you are pursuing a Master's Degree in Management, Environmental Science and Policy or Applied Leadership in Teaching and Learning.
- To apply to the Masters of Social Work program please follow the instructions on the MSW program web site.
- To apply as a Graduate Special use this Online Special Student Application. The special student classification is reserved for those students who want to take courses for credit but are not currently working toward a degree at UW-Green Bay.
An additional requirement for your application to the UW-Green Bay master of science program is three (3) Letters of Evaluation. Print THREE copies of this form. The completed forms should be mailed to our Admissions Office at the address indicated on the form.
