Application Procedure/Checklist for Dietetic Internship

Please follow all directions carefully as an incomplete application cannot be considered. Completed application and all supporting documents must be postmarked by February 15, 2008 for the Fall 2008 admission.

Send ALL materials (#1-6 on checklist below) in a single packet directly to:

1. $50 application fee (non-refundable). Make check/money order payable to UW-Green Bay
    
2. Completed Dietetic Internship application form (2005 version)
    
3. Letter of application
    
4. Official transcripts of credits from all colleges attended (transcripts must contain the official school stamp and issued in a sealed school envelope) 
    
5. One original copy of  ADA's "Declaration of Intent to Complete Degree" or "Verification Statement." One of  these forms is required to prove eligibility to apply for a Dietetic Internship
    
6. Three letters of recommendation (DEP reference letter form - each sealed/signed in a separate envelope) each with the "waiver of Recommendation Form"
