I am asking the IRB to waive the requirement that I document the subject's informed consent by obtaining a signed Informed Consent form for the following reason:
the only way that the subject could be linked to the research is through the Informed Consent form, and each subject will be offered the option of signing the Informed Consent form or not.
the research presents no more than minimal risk and involves procedures for which Informed Consent is not usually required outside the research context.
I will, however, provide each subject with a written statement regarding the research that includes all the information required to obtain Informed Consent.
