LibQUAL+ 2014 Survey

Thank you for participating in The Cofrin Library’s 2014 LibQUAL + survey. By taking this electronic survey, you can chart the course for the future of the Cofrin Library by weighing in on the services that the Cofrin Library provides to the UW-Green Bay community. Your feedback will be instrumental in understanding the perceptions and expectations of our campus community, in continuing or altering existing services and in designing services or expanding resources to meet a previously unnoticed need.

There is minimal risk associated with taking this survey. Furthermore, your participation is strictly voluntary, you can skip most of the questions at any time and you may withdraw your participation at any time. The information collected will be used by Cofrin Library and UW System to improve library services. Your responses will be kept confidential.

The survey will ask for your email address so that we can enter you in our drawing for prizes, however this email address is not connected to your responses in any way. You are not required to enter an email address and these will only be used to enter you in a drawing for the prizes and to contact you if you are selected for a prize. There will also be no connection between you and your specific responses in any future publication of the results.

Once the study is completed and the results compiled, the Cofrin Library will be sharing the resulting information with the UW-Green Bay community in several ways.
