NOTE: Your UWGB e-mail address is REQUIRED.  This helps us to know who has returned their survey so that we don't keep asking the same people for the same info.  YOUR NAME WILL NOT BE LINKED TO YOUR RESPONSES, AND YOUR NAME WILL NEVER APPEAR IN ANY REPORT OF THIS SURVEY'S RESULTS -- GUARANTEED. If you are not sure what your UWGB e-mail address is, here's the standard format: first 4 letters of your last name, first initial, middle initial, two-digit birth day. For example, let's say that John Q. Smith's birthday is March 26, 1983. His UWGB e-mail address would be: smitjq26@uwgb.edu.  The @uwgb.edu is standard for all UWGB e-mail addresses. If you're still not sure about your UWGB e-mail address, just put your name in the box.
