DIRECTIONS

Registration will be on-line (link is located at the bottom). Plan on 20-30 minutes. Please have the following information ready:

1. All schools, please have the following information:

List of student names, in 1-4 groups of 7-10 students, with t-shirt size (adult sizes, small-XXL) for each student
Number of lunches needed for teachers, paraeducators and students (lunch will be provided)

2. In-town schools only (Green Bay, West De Pere and Oneida) please have this additional information:

Dates/times of pre-visit and post-visit
A decision on attending the Professional Development Course

3. Permission slips are available to download below. We have also included translations into Hmong and Spanish for those who need them.

(Please return the permission slips to your point of contact teacher by OCTOBER 8.  These must be kept on file at UW-Green Bay to avoid any privacy misunderstandings as we ask for permission to photograph students for publicity purposes.)
