Audit Permission (e-form) (print form) form updated 3/12 Gain approval for audit only course registration. Special students only. Change Grading Status (print form) form created 10/11 Faculty approval for degree seeking students to audit or change grade status to pass/no credit. Course Registration Override - Waiver (e-form) (print form) Faculty approval to enroll by waiving closed course status, form updated 4/12 requisite requirements, instructor consent or time conflicts. Course Substitution (e-form) (print form) form updated 3/12 Faculty approval to use one course in place of another in your major or minor. (Gen Ed substitutions please use petition below). Declaration of Associates Arts and Sciences Degree (print form) updated 2/12 Declare Associate of Arts & Sciences degree with faculty approval. Declaration of Degree (e-form) (print form) form updated 1/12 Declare a major/minor with faculty approval. (If required, please complete your department's declaration documentation "BEFORE" completing this "Declaration of Degree" form.) Incomplete Grade (print form) form created 10/11 Used by faculty to submit incomplete grade deadline information. Instructor Approved Course Proposal for Registration (print form) form updated 3/12 Faculty approved Independent Studies, Internships, Teaching Assistantships, Project/Research Assistantships or Honors courses. Name Change (print form) form created 10/11 Submit documentation to change legal name of record. Replacement Diploma Request (print form) form created 10/11 Replace a lost or damaged diploma. Retroactive Credit (e-form) (print form) form created 10/11 Upon completion of a specific language course, additional credits can be earned with language coordinator approval. Transfer Course Equivalency Change Request (e-form) form created 10/11 Change a transfer equivalency to a direct equivalency of a UW-Green Bay course. Do NOT use this form for Course Substitutions. Petitions: (a request for waiver of a university rule or policy) Academic Forgiveness (print form) form created 10/11 Petition to remove prior academic grades from GPA calculation. Certain conditions apply to qualify. Late Add (e-form) (print form) form created 10/11 Faculty approval to add courses after the add deadline. Late Drop (e-form) (print form) form created 10/11 Petition to drop a course after the deadline for extenuating circumstances. Please direct all petitions to hruskaa@uwgb.edu.
