Forms & Petitions

Electronic forms require an active UWGB network user ID and password. If your network user ID is no longer active or if you don't have a valid UWGB network user ID, click on the form. You will have to create a user account at the log in page to complete the electronic forms.

Petition Forms

 

  Late Drop

Undergraduate/Graduate Students

Petition to drop a course after the deadline. Petitions are only granted for documented medical or military circumstances, or death of an immediate family member. The petition must address why the student was unable to drop the course prior to the deadline, and address how only this course is impacted and not all others. This form is routed to your class instructor. The Enrollment Review Committee will review the petition after the instructor completes the form. To petition for a medical reason, students must have their provider complete this Provider Verification form. If the request is due to the death of a family member, the petition must be accompanied by an obituary or copy of a death certificate. Petitions reviewed without the appropriate documentation will be denied. The student will receive an email notification from the Enrollment Review Committee after review.

 

  Late Withdrawal

Undergraduate/Graduate Students

Petition to drop all courses and withdraw from the University after the deadline. Petitions are only granted for documented medical or military circumstances, or death of an immediate family member. The petition and/or documentation must reflect why the student was unable to withdraw prior to the deadline. To petition for a medical reason, students must have their provider complete this Provider Verification form. If the request is due to the death of a family member, the petition must be accompanied by an obituary or copy of a death certificate. The Enrollment Review Committee will review the petition. Petitions reviewed without the appropriate documentation will be denied. The student will receive an email notification from the Enrollment Review Committee after review.

 

  Miscellaneous Petition

Undergraduate/Graduate Students ***This form is not used for any course registration transactions.

Petition to request an alternative or exception to an academic rule, regulation, procedure, or requirement of UWGB. GBOSS will forward the petition to the appropriate university adminstrator and communicate the approval or denial of their decision to the student via an email notification.
