Founders Award Nomination - Form

Each of these awards recognizes exceptional individual merit based typically on a cumulative record of sustained excellence rather than a single noteworthy achievement. Any member of the UW-Green Bay Community may make a nomination for any of the awards.

If the individual(s) you nominate is selected as a finalist, you will be contacted to provide further information such as background information on your nominee, letters of support addressing the nominee’s qualifications for the award, etc. All awards will be presented at the campus-wide convocation in August .
