Federal Direct Loan Entrance Counseling
- First time borrowers must complete on-line entrance counseling for their loan to disburse. Entrance counseling is designed to help you understand your Direct Loan responsibilities and takes about 20 minutes to complete.
Federal Direct Loan Master Promissory Note (MPN)
- First time borrowers must complete an on-line MPN for their loan to disburse. The MPN describes the terms and conditions of the loan and asks for personal information and references. The MPN is good for 10 years.
Perkins Entrance Interview & Master Promissory Note (MPN)
- If you have been awarded a Perkins Loan for the first time at UWGB, you'll need to complete the on-line entrance interview and MPN for your loan to disburse. The Perkins Entrance Interview and MPN is a one step process.
PLUS Loan Application
- To offset college costs, your parents may want to consider applying for the Parent PLUS loan. If your parents are approved for the Plus loan, they can borrow funds for your education (up to the cost of education, minus any financial aid). The loan is in the parent’s name and the interest rate is 7.9%.
If your parents are denied the PLUS loan you, the student, may be eligible for additional unsubsidized loans. The Financial Aid office will review the denial and offer additional unsubsidized loans if eligible.

Additional Expense Form
- The financial aid office may be able to consider additional expenses, when calculating financial aid awards, by submitting this form along with supporting documentation.
Reduced Income Form
- The financial aid office may be able to consider a reduction in income, when calculating financial aid awards, by submitting this form along with supporting documentation.
Consortium Agreement
- A consortium agreement recognizes the enrollment of a student at multiple colleges and certifies that only one college will be providing financial aid. Use this form if you are attending UWGB and taking credits at another college, which will transfer in to fulfill requirements towards your degree here.
