Brief Guidelines for Inserting Footnotes (in MS Word)
Use the Hacker manual, pages 193-202, or Rampolla, pages 87-102, for models for footnote formats. Your first citation should include the full publication information of the work. You do not need a separate bibliography.
Do not do the numbering manually. Instead, use the "Insert Footnote" function in Word (or your word processor).
To do this, go to the "Insert" menu and select "Footnote."
You should now see the following window, which allows you to choose either footnotes or endnotes--either is fine.
Make sure that you are using Arabic numerals (1, 2, 3, etc.)--if you are not, you can use the "Options" button to change this.
