Survey Service - Terms of Use
The University of Wisconsin – Green Bay provides a service for developing and hosting online surveys. The service is provided to users subject to the terms and conditions set forth in these Terms of Use. Please read the following terms carefully. By using the service, users accept and agree to all of the terms and conditions set forth is this document.
- Users creating and administering surveys personally accept full legal responsibility for these surveys. This includes but is not limited to, legal responsibility for wording of questions on the survey, procedures used for conducting the survey, and appropriate security of data collected by the survey. Non-University employees and/or others not acting on behalf of the University, agree to indemnify the University from and against any and all claims and expenses, including attorneys' fees, arising from the use of this service. This expressly includes responsibility for any and all liability arising from the violation or infringement of proprietary rights or copyrights and any libelous or unlawful material contained within the survey.
- Users agree to obtain all appropriate approvals from the Institutional Review Board and any other appropriate agencies before conducting the survey. (The IRB must review any research protocols involving human subjects in which the data collected will be published, used in a poster presentation, used in an oral presentation or maintained in a database for later use. APPROVAL MUST BE OBTAINED PRIOR TO ANY DATA COLLECTION.)
- Users must review and abide by all University policies, including but not limited to Employee Acceptable Use Policy for Technology and the Internet, Ownership, Use and Control of Copyrightable Instructional Materials, and E-mail Policy & Guidelines, Distribution List.
- Users agree to never ask participants for the following information:
- Credit card or bank card numbers
- Personal identifiable information (i.e. social security numbers, birthdates)
- Passwords to any service on- or off-campus
- Information which participants cannot legally divulge.
- Users agree to protect the confidentiality and privacy of survey participants and responses provided.
- Users are responsible for the creation and operation of the survey. Students conducting a survey must have a faculty or staff sponsor.
- Users are expected to provide support for participants in the survey and are required to provide the appropriate contact information if participants have questions. Information Services will not provide assistance in interpreting questions or with problems resulting from the construction of the survey.
- The University will provide the online survey service, but does not accept responsibility for how individuals use the service, network failures, delays or other technical problems.
- The University will provide an access account for a survey not an individual user. The user is required to notify the Survey Tool Administrator when a survey will launch and when it will conclude. Access to the online survey service will be terminated after an appropriate amount of time.
- While the University agrees to keep survey data for two years, it is the user’s responsibility to extract the data for long term storage and backup.
- The University is authorized, but is not required, to monitor usage within the online survey service and has the right to remove, revise or censor content without notice at its discretion for any reason.
- The user acknowledges and agrees that the University may, with or without notice, modify or terminate the online survey service at any time.
