New Hire Paperwork

In order to ensure accurate information is on file and for timely processing of payroll, it is very important that within your first week of employment, you log into the My UW Portal to enter/update the following information. Delaying submission of new hire paperwork will postpone access to email and other campus systems.

Be sure to let your supervisor know the date these items are completed, as this information will be noted on your Onboarding Checklist. If you have any questions on the completion of the forms, please contact Human Resources at 920-465-2390.

Please note that if you already submitted Direct Deposit, W-4 and Self-ID forms, you would not need to add this information in your My UW Portal, but can update it as needed.

  • I-9 Form

You will receive an email from HireRight requesting your action to initiate the I-9. This email will provide you with logon information to complete your I-9. Please follow the instructions in the email to complete page 1 of the I-9, if you haven’t already.  The I-9 form contains a list of acceptable documents you will need to physically present within three business days of your first day of employment for verification of identity and work authorization. 

If you haven't already, please submit the following.

  • Faculty and Academic Staff - Submit official transcripts for your highest degree.  Unofficial transcripts are acceptable if you are a University of Wisconsin-Green Bay graduate.
University Staff – Submit unofficial transcripts for your highest degree.
