The disruption caused by COVID-19 has been difficult for all of us. It continues challenging us to co-exist with an unpredictable virus while we simultaneously pursue our educational goals and keep ourselves and each other healthy and safe. This Phoenix Forward website includes important information about COVID-19 testing, as well as safety measures and campus operations for how the University will function during the pandemic and what is required of the entire campus community to operate safely. UW-Green Bay continues to follow the advice of local public health, the Centers for Disease Control and our partner, Prevea Health. Please takes measures to protect yourself and the Phoenix Family. Vaccination against COVID-19 of faculty, staff and students, continues to be the best way for the University to return to the pre-pandemic social and academic environments.

Vaccination Verification

Those students and employees who are fully vaccinated and upload documentation via the Exemption Request Forms linked below no longer will be required to participate in surveillance testing or maintain six feet of physical distance and can return to campus without restrictions post-travel.
