Vaccination Verification

Students, employees and visitors who are vaccinated are not required to participate in surveillance testing or practice social distancing while in university buildings. Those with exempt status can return to campus without restrictions post-travel. Additionally, through the "70 in a Phlash" initiative, vaccinated students that provided vaccination verification are eligible to win prizes such as Pass Points, a Brewers Experience, a Packers Experience, scholarships and more.

UW-Green Bay continues to follow the advice of local public health, the Centers for Disease Control and our partner, Prevea Health. Please takes measures to protect yourself and the Phoenix Family. Vaccination against COVID-19 of faculty, staff and students, continues to be the best way for the University to return to the pre-pandemic social and academic environments.
 

Fully vaccinated

An individual is considered fully vaccinated:
  • Two (2) weeks after receiving both doses of a two-dose vaccine (Pfizer or Moderna)
  • Two (2) weeks after receiving a one-dose vaccine (Johnson & Johnson)

In order to receive exemption and be entered to win through "70 in a Phlash" initiative, documentation of vaccination(s) should be uploaded via the Exemption Request Form. Vaccination verification can include a legible image of a personal vaccination card (including name and date of birth) or an official record obtained from the Wisconsin Immunization Registry.

Employee Instructions

Employees who are fully vaccinated can get an exemption from COVID-19 protocols by completing the Immunization Record form located in the MyUWPortal. As part of the form, employees will be required to upload proof of vaccination, by either providing a copy of their vaccination record from the Wisconsin Immunization Registry or a copy of their vaccination card (with name and date of birth).

Confidentiality

Vaccination Status is Confidential. UW-Green Bay also maintains vaccination records confidentially. It should be presumed that an individual who is unmasked has provided the necessary verification through the appropriate processes, and no one should request or require individuals to disclose their vaccinations status. Individuals of concern should be reported to the Office of Human Resources or Dean of Students as applicable and the assigned office will follow up on the concern.

Pop-up vaccination clinics

Appointments for pop-up vaccination clinics are appreciated and may be made through MyPrevea.com or 1-833-344-4373, but walk-ins are welcome. Vaccines are available to students, staff, faculty, and family members (ages 12 and up). Please bring your insurance card, if applicable, but insurance is not required.
