Daily Self-Assessment Tool

Students living in Residence Life are required to self-assess and self-report on a daily basis.
Commuter students are required to self-assess and self-report prior to each day they come to campus for classes, appointments or any other on campus activities.

On days that you have class and work on campus as a student employee, it is only necessary to complete the student self-assessment below. On days that you only work as a student employee on campus, please complete the employee assessment.
