Administrator Training Program Application
Organizations wishing to submit an Assisted Living Administrator's Training program may complete the program application. Steps to complete the application are below. The following information will be needed during the application process:
- Program name, address, e-mail and phone
- Contact person
- Instructor information including name, date of birth, phone, e-mail, qualifications and subject(s) they will teach
Step One
Complete the program application and submit payment. Please note that fees are not refundable.
Step Two
Upon completing the application, curriculum will need to be forwarded to the registry.
Step Three
After processing the program application, this registry will send the primary contact an e-mail with a customized link for the instructor information to be uploaded.
