Accident ReportReport an event that has occurred relating to any personal injury, property damage, fire, chemical spill, acts of negligence, MVA's, or other incidents not related to Sexual Assault, Bias/Hate Crime using the Accident Report portal. This report is not intended for workers compensation claims. Use the Workers Compensation page.
Bias Incident or Hate Crime ReportReport incidents motivated by bias or hatred, which occur, on and off the UWGB campuses using Bias Incident Report.
Bloodborne Pathogen ExposureExposure to bloodborne pathogens should be reported immediately to a Supervisor. Exposure can be any stick or direct contact. Employees are encouraged to seek testing and submit testing invoice to UW-Green Bay Compliance Specialist who will review and approve.
Safety ConcernReport safety concerns to the Safety & Environmental Manager through the Safety Concern portal.
Student ComplaintStudents have the right to seek resolution when they believe they received unfair or improper treatment from a UW-Green Bay faculty or staff member, department or office. Students are encouraged to resolve the matter informally before initiating the formal complaint process with the Office of the Provost.
Title IX Sexual Assault ReportingAn individual who believes they are a victim, or who have received a report of or witnessed an incident of relationship violence or sexual harassment may report the incident to the Title IX Coordinator.
Volunteer FormIndividuals serving at the University as a general volunteer should complete a Volunteer Agreement. Individuals serving in a Covered Activity as part of a youth program should work with their Program Facilitator in completing the Sponsored Staff Acknowledgement.
