Applying for Academic Accommodations

Student Accessibility Services (SAS) has a short, three-step application process to receive academic accommodations. Once we have received your application and disability documentation, you will meet with SAS staff to determine reasonable accommodations. Because accommodations are not retroactive, we encourage you to apply as soon as possible.

1 Complete the Application

  • Log in to GB Access with your UW-Green Bay email and password.

  • Complete the application form. Please fill in the form as completely as possible.

2 Submit Documentation

Next you will need to provide documentation of your disability(s), to show that you are eligible for support. It may be a medical, psychological or neuropsychological evaluation. An Individualized Educational Plan (IEP) could be submitted if the diagnostic information and the last IEP evaluation is included. Documentation is medical information that must be provided by a treating professional and needs to have a diagnosis of disability and how it affects you daily.

  • If you already have documentation, you can upload it as part of the application in GB Access, email it to SAS or bring it in to our office and we will add it to your application.
  • If you don't have documentation now, you can upload it to GB Access or bring it in, later.
  • If you need forms for your treating professional to fill out, we have some you can use on our documentation page.

3 Schedule an Intake Appointment

Once you have filled out the application and uploaded documentation, SAS will send an email to your secure UW-Green Bay email account about needed information, next steps and scheduling appointments.
