Graduate Forms
Dynamic Forms (e-forms) require a network ID and password. If your network ID is no longer active, each e-form will give you the opportunity to create an ID to complete the form.

Applications
Ready to apply? Here's where to start.
- Apply to UW-Green Bay Graduate Programs: Start the application process to take a graduate course, certificate, or degree.
- Graduate Certificate Declare Form: For currently admitted degree-seeking students who would like to apply for a certificate program
- Application for Graduation: Required of all graduate degree-seeking students, and due one semester prior to their expected graduation term.
- Discontinued Student Re-admit Form: For UW-Green Bay graduate students admitted into a degree program, have not taken a course for one to three semesters, and are ready to return to their program.
- Students who have been away from the university for four or more semesters must reapply to their program.
- Approval for Admission to an Accelerated Track (GR-A): For UWGB undergraduate students requesting enrollment in cross-listed courses for graduate credit in one of our Accelerated Undergraduate Majors.
