Qualifications

Instructor applications are processed through this website. Please take a moment and read the directions for each type of instructor before applying. If you have any questions, please e-mail registry@uwgb.edu.

Prior to starting an application make sure that you can answer "yes" to the following requirements and have documentation for qualification (if required) ready to upload.

Fire Safety Qualifications

  1. I have knowledge of Wisconsin Administrative Code for community settings, and can describe how I gained this knowledge.
  2. I acknowledge at least oneof the following:
    • I have successfully completed at least six hours of instruction related to teaching or training adults and have documentation of this training.*
    • I have at least two years of experience teaching or training adults and have a reference who can verify this experience.
    • I am a current American Red Cross or American Heart Association Instructor and have my ID number available.
  3. I acknowledge at least oneof the following:
    • I am certified by the Wisconsin Technical College System Fire Service Certification Program (minimum Fire Fighter 1.)
    • I am an instructor employed by a Wisconsin Technical College to teach at least one course within an accredited Fire Safety curriculum. College name and human resource contact will be required during the application process.
    • I am on the Training Registry as having completed a department-approved train-the-trainer course in Fire Safety. Find a course.
    • I have at least two years of full-time work experience in a health care setting, I am on the Training Registry as having completed a Fire Safety course and I have retained a person certified by the Wisconsin Technical College System Fire Service Certification Program (minimum First Fire Fighter) to act as my training consultant. Please be prepared to provide the full name, certification number and contact information of consultant. I understand that the consultant must observe and complete a written evaluation of the first class I teach under this approval and one training annually thereafter.
  4. I am prepared to pay the $115 fee to become an instructor. A credit card payment will be accepted during the online application process.

Once the application is completed, you will be notified (if applicable) of additional information that might need to be provided by e-mail. Returning these items quickly will speed up the approval process. You will be notified via e-mail of the approval decision. Once approved, you will be added to the registry of approved instructors and will have access to the required training materials via your Instructor Dashboard. Approvals are valid for a maximum of two years.

First Aid and Choking Qualifications

  1. I have knowledge of Wisconsin Administrative Code for community settings, and can describe how I gained this knowledge.
  2. I acknowledge at least oneof the following:
    • I have successfully completed at least six hours of instruction related to teaching or training adults and have documentation of this training.*
    • I have at least two years of experience teaching or training adults and have a reference who can verify this experience.
    • I am a current American Red Cross or American Heart Association Instructor and have my ID number available.
  3. I acknowledge at least oneof the following:
    • I am on the Training Registry as having completed a department-approved train-the-trainer course in First Aid and Choking. Find a course.
    • I am a physician, registered nurse, licensed practical nurse, nurse practitioner, physician's assistant, fire fighter 1, EMT, paramedic or an instructor at a Wisconsin Technical College teaching at least one course in First Aid and Choking.
  4. I am prepared to pay the $115 fee to become an instructor. A credit card payment will be accepted during the online application process.

Medication Administration Qualifications

  1. I have knowledge of Wisconsin Administrative Code for community settings, and can describe how I gained this knowledge.
  2. I acknowledge at least oneof the following:
    • I have successfully completed at least six hours of instruction related to teaching or training adults and have documentation of this training.*
    • I have at least two years of experience teaching or training adults and have a reference who can verify this experience.
    • I am a current American Red Cross or American Heart Association Instructor and have my ID number available.
  3. I acknowledge at least oneof the following:
    • I am a physician, registered pharmacist, registered nurse, physician’s assistant or nurse practitioner. Be prepared to enter license number during application process.
    • I am on the Training Registry as having completed a department approved train-the-trainer course in Medication Administration and an instructor employed by a Wisconsin Technical College to teach at least one course in Medication Administration. College and human resource contact number will be requested during application process.
    • I am on the Training Registry as having completed a department approved train-the-trainer course in Medication Administration and I have retained a licensed professional in good standing who will act as a consultant for medication training issues, concerns and questions. I understand that the consultant must observe and complete a written evaluation of the first class I teach under this approval and one training annually thereafter. Find a course.
  4. I am prepared to pay the $115 fee to become an instructor. A credit card payment will be accepted during the online application process.

Standard Precautions Qualifications

  1. I have knowledge of Wisconsin Administrative Code for community settings, and can describe how I gained this knowledge.
  2. I acknowledge at least one of the following:
    • I have successfully completed at least six hours of instruction related to teaching or training adults and have documentation of this training.*
    • I have at least two years of experience teaching or training adults and have a reference who can verify this experience.
    • I am a current American Red Cross or American Heart Association Instructor and have my ID number available.
  3. I acknowledge at least one of the following:
    • I am a physician, licensed practical nurse, registered nurse, physician’s assistant, nurse practitioner or respiratory care practitioner. Be prepared enter your license number.
    • I am certified in Infection Control and Epidemiology and my certificate is valid and in good standing. Be prepared to submit a copy of your certificate via e-mail after the online application is complete.
    • I am an instructor employed by a Wisconsin Technical College to teach at least one course in Standard Precautions. College name and human resource contact will be required during the application process.
    • I am a currently authorized American Red Cross instructor, in good standing, of an American Red Cross Bloodborne Pathogens course.
    • I am on the Training Registry as having completed a department-approved train-the-trainer course in Standard Precautions. Find a course.
    • I have at least two years of full-time work experience in a health care setting, I am on the Training Registry as having completed a Standard Precautions course and I have retained a licensed professional in good standing who will act as a consultant for standard precautions training issues, concerns and questions. Be prepared to provide name, license number and contact information of consultant. I understand that the consultant must observe and complete a written evaluation of the first class I teach under this approval and one training annually thereafter.
  4. I am prepared to pay the $115 fee to become an instructor. A credit card payment will be accepted during the online application process.

Please Note

The six hours of instruction related to teaching or training adults must be a class that is specific on how to teach adults. Watching someone teach a class does not count. The class should be documented with a certificate of attendance or completion.
