Financial Aid Forms
Need to submit a document related to your financial aid? You can find it here.
FAFSA
FAFSA stands for "Free Application for Federal Student Aid." The FAFSA is available October 1 for the following academic year. You must apply each year you're seeking aid. Our FAFSA Federal school code is 003899.
Verification Forms
Not all financial aid applicants get selected for verification, but some do. It's nothing you did wrong, it's just something the U.S. Department of Education requires.
Appeal Forms
Maybe you lost your aid. Maybe you need more aid. Perhaps you or a parent are facing a substantial income reduction. Or maybe your college costs changed. These appeal forms will allow you to request special consideration for your aid eligibility.
Loan Requirements
After you accept a federal direct student loan, you need to complete entrance counseling and sign a master promissory note.
Report Outside Aid
If you are receiving other sources of educational assistance (scholarships, veteran's benefits), you must report them to the Financial Aid Office.
Consortium Agreement
If you're planning to graduate from UW-Green Bay but are taking classes at another school, you need a Consortium Agreement to receive financial aid for the credits you transfer.
Pay Permissions
This one-time authorization will allow your financial aid to pay for most (not all) charges found in SIS, like your books!
Summer Aid Application
For consideration of summer loans or summer work study, complete the newest FAFSA and this summer aid application, available in March of each year.
Secure File Upload
Sometimes students need to send us documents with sensitive information. We have a secure file uploader just for your financial aid documents.
