Course Description

With the principles of wraparound as a foundation, care coordinators and services facilitators will learn how to develop a team and guide them through the wraparound process including completion of a strength-based assessment, consideration of underlying needs, development of an individualized plan of care including planning for crisis, and facilitation of transition out of the formal team process.

Learning Objectives: 

  • Enhance skills necessary to collaborate with partners, youth, and families to address multiple needs within the context of the system of care.
  • Enhance skills related to team building, including trust-building and building relationships with youth and families.
  • Enhance skills related to care coordination and team meeting facilitation.
  • Facilitate an assessment process that reflects the family’s long-term goal(s), culture, underlying needs, strengths, and youth and family supports.
  • Develop a functional strength-based plan of care, including planning for crisis.
  • Understand the team decision-making process and basic conflict management skills.
  • Enhance skills related to facilitating a team’s transition out of the team process/closure.

About the Trainer

Jessica Beauchamp, LCSW, SAS, SOTS

Jessica Beauchamp, LCSW, SAS, SOTS

Jessica Beauchamp has 18+ years of experience in the human services field. She earned her graduate degree from Michigan State University. Areas of knowledge includes case management, service facilitation, administration, management, counseling, and program development. Jes worked in Community Mental Health (CMH) agencies for several years and in 2016 opened her own private mental health practice.
With CMH she provided crisis intervention services and worked closely with the court system and Chapters 51 and 55. She also worked in several roles within Comprehensive Community Support (CCS), Community Support Teams (CST), and Children’s Long-Term Services (CLTS) programs.

In private practice Jes offers unique services in that a majority of the treatment takes place in the community, nature, or the client home. She also contracts and consults with local practitioners and several agencies around the state of WI.

Jes has presented to a variety of audiences on topics including: wraparound, assessing suicide risk, crisis/safety planning, substance use/abuse, ethics/boundaries, personality disorders, Autism and HIPAA.

Adam Graham, LPC-MHSP

Adam Graham, LPC-MHSP, loves helping interdisciplinary groups find new solutions to challenging problems. From the medical, psychological, recovery, and law enforcement communities to government to the private sector, Adam has gained invaluable experiences and insights that, in dialogue, can move mountains to serve people experiencing a mental health crisis. Trained as a clinical therapist, Adam has served these groups in his 14 years in the community mental health system as a clinician and emergency psychiatric services director, as well as in the roles of professor, threat assessment consultant to federal law enforcement, and international volunteer aid worker. Based in Nashville, TN, he joyfully fulfills the local obligation to be a musician, as well as an experienced trainer, presenter, problem-solver, and leader.
