Fundamentals of Wraparound
For Care Coordinators and Service Facilitators
With the principles of wraparound as a foundation, learn how to develop a team and guide them through the wraparound process including completion of a strength-based assessment, consideration of underlying needs, development of an individualized plan of care including planning for crisis, and facilitation of transition out of the formal team process.
