Declaring a Major or Minor
Students can declare a Major or Minor at any time through a simple on-line process. Please complete the following steps before submitting your form.

2. Complete the Declaration e-form. Once approved or denied you will receive an email. If approved, the Student Services Center will update your record. You will then see the new program requirements and advisor in your SIS account.
Electronic forms require an active UWGB network user ID and password. If your network user ID is no longer active or if you don't have a valid UWGB network user ID, click on the form. You will have to create a user account at the log in page to complete the electronic form.
