To earn your master's or doctoral degree, you will occasionally need to update your academic record, change your schedule, or submit official requests using the forms listed below. Most are Dynamic Forms (e-forms). When you fill one out, it automatically routes to your advisor, instructors, or the graduate office for digital approval.
Enrollment Forms
Discontinued Student Re-admit Form
- Purpose: Resume your graduate program after a brief break.
- For: Admitted students who haven't taken a class for 1 to 3 terms.
- Not For: If you’ve been away for 4 or more terms, you must complete a new application to your program instead. See Graduate Admissions.

Accelerated Master's Program (AMP) Forms
These forms are exclusively for current UW-Green Bay undergraduate students pursuing early master's program credits.
Accelerated Track Admission
- Purpose: Apply for entry into an Accelerated Master's Program (AMP)
- For: Eligible UW-Green Bay undergraduate students. Refer to the Graduate Catalog for Accelerated Master's Program requirements.
- When: Junior year and when program requirements are met
- Approval: This form goes first to your recommending faculty member, then routes through your Faculty Mentor, the Graduate Program Chair, and the Graduate Studies Office before reaching your Professional Advisor to officially update your record.
Accelerated Course Enrollment
- Purpose: Request approval to register for graduate-level courses for graduate credit.
- For: Current UW-Green Bay undergraduate students already formally admitted into an Accelerated Master's Program (AMP).
- When: You must complete this every semester you intend to enroll.
- Approval: Requires Advisor and Graduate Studies Office approval.
Declaring a Program
Use these forms to declare your academic goals.
Declaration of Master's Degree Track
- Purpose: Officially establishes your specific master's program track or emphasis in the university system, so your academic record is set up correctly.
- For: Degree-seeking graduate students who want to declare a track or emphasis.
- When: Complete this as soon as you are admitted and ready to move forward.
Declare Master's Degree Track/Emphasis
Graduate Certificate Declaration
- Purpose: Adds an official certificate credential to your active degree track.
- For: Currently-enrolled, degree-seeking graduate students.
- Not For: Already graduated, or want a standalone certificate? Don't use this form—instead, apply as a Graduate Special student.
Registration Forms
Use these forms when registering for special coursework.
Course Registration Override / Late Add Form
- Purpose: Request a waiver to bypass registration roadblocks like missing prerequisites, time conflicts, closed classes, or adding a course late.
- For: Any graduate student experiencing a standard registration error or delay.
- Approval: This form routes directly to your course instructor for review.
Graduate Assigned Study
- Purpose: Request graduate credit for a course that isn’t a standard graduate offering (most commonly used to elevate a 300- or 400-level undergraduate course for graduate credit). See the Graduate Catalog for more about Graduate Assigned Study.
- For: Enrolled graduate students who want to substitute a required course with an undergraduate course that better fits academic background or goals, with adjustments for academic rigor.
Independent Study Form
- Purpose: Request official registration for individualized, supervised credit-bearing experiences (independent research or projects).
- For: Enrolled students completing academic work outside the standard class schedule.
Internship / Co-op Form
- Purpose: Secure required academic credit for professional, hands-on field experience.
- For: Enrolled students whose programs carry a formal, credit-bearing internship requirement.
Requesting Academic Exceptions
Credit Overload Petition
- Purpose: Request permission to take more than 15 graduate credits in a single Fall, Spring or Summer term.
- For: Enrolled graduate students.
- Exception: Officially designated travel courses are exempt and do not require this petition.
Course Substitution Request
- Purpose: Request approval to swap a required course in your graduate program with an alternative graduate course that better fits your background or goals.
- For: Enrolled graduate students.
Late Withdrawal Form
- Purpose: Petition to withdraw from all coursework after the standard deadline due to severe, unavoidable circumstances.
- For: Students facing documented medical emergencies, mental health crises, military deployment, or the death of an immediate family member. For medical or mental health petitions, your licensed care provider must fill out the Provider Verification form.
- When: Must be filed within one year of the requested semester.
Special Petition General Form
- Purpose: Requests a variance or exception to any standard academic rule, regulation, or procedure not covered by another form.
- For: Any graduate student facing a highly unique academic constraint or situation.

What if I don't have a UWGB Login?
If you don't have an active UW-Green Bay login, you can create a guest ID from the form's login screen to access and submit your paperwork. Click the link that reads "Create a separate forms login to complete this form electronically."

Graduate Studies forms are completed through a secure electronic form system that normally requires a UW-Green Bay network ID and password. Former students whose accounts are no longer active can use the guest account option to complete required forms.
Will I receive confirmation that my form was received?
Yes. When you submit a form on this page, you should receive immediate confirmation that your form was successfully submitted. Most of the registration forms are submitted through UW-Green Bay’s secure Dynamic Forms system and include routing to faculty and staff for approval and implementation steps. After you submit a form, the system provides an on‑screen confirmation indicating that your form was received. Some forms may send a copy or notification to your UW‑Green Bay email. What qualifies as a valid reason for a late withdrawal?
A valid reason for a late withdrawal is an extenuating circumstance that occurred during the semester being petitioned and prevented you from continuing your coursework or graduate program. Examples of valid reasons include:
- A serious physical or mental health condition, verified by a physician or licensed counselor. A Provider Verification Form is required.
- A death or prolonged serious illness of an immediate family member, verified by an obituary, physician's statement or other official documentation.
- Military activation or orders that prevent you from completing the semester. Supporting documentation is required.
To request a late withdrawal, you must notify your Graduate Advisor and instructors, submit the Late Withdrawal form and supporting documentation, and file the petition within one calendar year of the semester being petitioned. All petitions are reviewed by the Enrollment Review Committee. Additional information is available in the Graduate Catalog under Petition Process for Late Drop or Withdrawal.
