Apply for Housing Accommodations

The Student Accessibility Services Office (SAS) is the designated entity on campus to collect all documentation as it relates to disability-related accommodations, including for students living on campus with Housing and Residential Education in residence halls or apartments. Decisions about housing accommodation requests are made by committee. Students interested in applying for a housing accommodation should complete the following steps:

1 Complete a Housing Contract

Go to your Housing Hub to complete the housing application. If there are questions about this process, please contact Housing & Residential Education at housing@uwgb.edu.

2Complete Accommodations Application

Log into the GB Access System to complete the form and upload the required documentation. NOTE: Not completing all forms or all sections of the form will delay a decision.

  1. Student Disability Accommodation Request form (pdf)
  2. Medical Documentation for Housing Accommodation (pdf)
  3. Medical-Documentation for ESA (pdf - only submitted if applying for an Emotional Support Animal (ESA) accommodation)

3Schedule a Meeting

If interested in doing so, students are able to schedule a meeting with a housing staff member to review the accommodations process. The student can make the request by emailing housing@uwgb.edu. A housing staff member may also reach out to schedule this meeting.

4Review of Request

After all documentation is received, it will be reviewed by a committee of staff from across campus.

5Decision

After review of the request, a decision regarding the request and all accommodations will be sent to the student’s UWGB email account. Please continue to monitor your email for this decision.

Priority Application Deadlines: The accessible housing application and approval process can take several weeks. In order to best meet student needs, it is recommended that applicants complete this process as soon as possible. Students who complete their application by the priority deadline each semester will ensure approved accommodations are made prior to move in. After this priority deadline, applications will be reviewed as time allows.

  • August 1st for Fall Housing Move In
  • December 1st for Spring Housing Move In
  • May 1st for Summer Housing Move In
