Late Fee Appeal Form

It is assumed that you are aware of the tuition fee schedules, late payment penalties, refund schedules, and deadlines.

If there are questions about the accuracy of any terms or if you are seeking payment arrangements see the Student Billing Office, Room SS1300, before completing this form.

Late Fee Appeals will automatically be denied for the following reasons:

• The term has ended
• The past due balance has not been paid in full
• You have signed up for the Payment Plan after the due date (effective Summer 2026)
• You did not complete financial aid applications or requirements in a timely manner
