Substitution Forms

Undergraduate/Graduate Students
Faculty approval to use one course in place of another in your Major or Minor. The student will propose a course substitution and forward the eform to their faculty advisor for approval. Students will get an email notification of approval or denial once the advisor completes the form. If approved, the Registrar's Office will update SIS. Students will then see the substitution reflected on their academic requirements in their SIS account.

 


For transfer courses, courses that are not on the approval gen eds list, travel experiences etc.
Associate Dean of Liberal Arts and Sciences approval to have a transferred in course  or travel experience approved to fulfill a General Education requirement. This form can also be used to request approval for an alternate course to be used in place of the approved General Education list of courses. Students will then see the substitution reflected on their academic requirements in their SIS account.

 



Used by students to request a UW-Green Bay General Education course be moved from fulfilling one category of General Education to another. The Registrar's Office will review the form and make the change on the student's SIS account if necessary.
